Telemedicine. Is it more used in rural areas or in cities?

If we consider the data from the latest report from the National Telecommunications and Information Administration and the Economics and Statistics of the United States, Americans use to:



1.- Job search, 73% of the unemployed in the U.S. looking for work in this way.
2.- Citizen participation.
3.- Health research and finding information in general, but also used as a vehicle for professional interaction and more innovative telemedicine.
4.- Other activities as social network use, blogging, etc..

We can see that Health would be a Trend Topic on the needs of American citizens regarding the use of the Internet.



Although telemedicine is offered as a tool for communication between patients and professionals and provides unique opportunities for the treatment and prevention of chronic conditions in rural or remote medical centers of reference, this report shows that it is more prevalent use in urban populations, at least in the U.S.



The base of this report include data from 53,000 households during the month of July 2011. In that we can to find that Internet users of urban areas were more participants in this type of medicine, than users of rural areas, utilization rates were 8% and 4% respectively. The 7% of Internet users reported that connect to the network, to access medical records, participate in a video conference with the health professional or remote procedures as heart rate monitoring. The data show disparities in income and education level, but few racial disparities. 11% of users with incomes over $ 100,000 has devoted time to activities related to telemedicine and only 4% of users with incomes below $ 25,000 have reported such use.



If we analyze this report with details, we can conclude that bring telemedicine to rural areas is a government decision as part of a health program, and pursues the objectives of bridging the gap by using this technology, reducing costs and making life easier for users in a large number of specialties, not replace traditional medicine, and come through the use of this technology to urban users, enable an optimization of health services that will be accessible in a way more functional and more comfortable for many users. What it is clear from this report is that telemedicine is a trend of investment, development and use and is not intended to replace anything, just provide tools to optimize health services offering the same in a more accessible , more functional, with less cost and saving all kinds of distances, physical, cultural, economic, etc..

Telemedicina ¿Mundo rural?¿Mundo urbano?

Si tenemos en cuenta los datos del último reporte de la Administración Nacional de Telecomunicaciones e Información y la Administración de Economía y Estadística de los Estados Unidos, el uso que los norteamericanos de Internet se puede clasificar de la siguiente manera:



1.- Búsqueda de empleo, el 73% de los desempleados en EEUU buscan trabajo por este medio.
2.- Participación ciudadana.
3.- Salud, investigación y búsqueda de información en general, pero también se utiliza como vehículo de interacción con profesionales y de forma más novedosa la telemedicina.
4.- Otras actividades, redes sociales, blogging, etc..

Podemos observar que la Salud, se encuentra en lo que sería un Trend Topic, en las necesidades de los ciudadanos americanos en cuanto a la utilización de Internet.



Aunque la telemedicina se ofrece como una herramienta que permite la comunicación entre pacientes y profesionales y que brinda oportunidades únicas para la atención y prevención de dolencias crónicas, en entornos rurales o alejados de los centros médicos de referencia, este informe nos muestra que es más frecuente el uso en poblaciones urbanas, al menos en EEUU.



La base de este informe son los datos obtenidos de 53.000 hogares durante el mes de Julio de 2011, donde se comprobó que los usuarios de Internet que vivían en zonas urbanas eran más participes de esta modalidad de medicina, que los usuarios de un ámbito rural, las tasas de utilización fueron de un 8% y 4% respectivamente. El 7% de los usuarios de Internet informó que se conectan a la Red, para acceder a los registros médicos, participar en una videoconferencia con el profesional sanitario o para procedimientos remotos, como monitorización en tiempo real. Los datos muestran disparidades en cuanto a  los ingresos y en el nivel educativo, pero pocas disparidades raciales. El 11% de los usuarios con ingresos superiores a 100.000$ ha dedicado tiempo a actividades relacionadas con la telemedicina y sólo un 4% de los usuarios con ingresos inferiores a 25.000$ reportaron ese tipo de uso.



Si analizamos este informe detalladamente podemos concluir que llegar mediante el uso de la telemedicina a entornos rurales y con menos recursos es una decisión gubernamental como parte de un programa sanitario, que se plantee como objetivos el acortar las distancias mediante el uso de esta tecnología, reduciendo costes y facilitando la vida a los usuarios en un número muy elevado de especialidades, sin reemplazar la medicina tradicional, y llegar mediante el uso de esta tecnología a los usuarios del entorno urbano, posibilitará una optimización de los servicios sanitarios que serán accesibles de una forma más funcional y más cómoda para muchos usuarios. Lo que si queda claro en este informe es que la telemedicina es una tendencia de inversiones, de desarrollo y de uso y que no se pretende sustituir a nada, simplemente facilitar unas herramientas que permitan optimizar los servicios sanitarios ofreciendo los mismos de una forma más accesible, más funcional, con menos costes y salvando todo tipo de distancias, físicas, culturales, económicas, etc..

Telemedicine in prisons, an example of rentability

One of my good friends in all his talks about telemedicine, he said "Telemedicine is not a luxury, it is a need and sometimes an obligation".



Doc. Claudio Pelaez is a great defender of telemedicine and during ten years, he was the Regional Coordinator of Telemedicine Program in Extremadura. He has developed telemedicine,  all than he could or have allowed him. This telemedicine program began in 2002 with first pilot program and Claudio Pelaez and all his team have developed and configured a work protocol and they have won experience, they have created a new medical model based in telemedicine with a satisfaction rates above to 90%.

One of the bets more interesting about telemedicine in Extremadura was to bring telemedicine systems to prisons and to allow to them access to medical services across the technology.



In US, Texas Deparment of Criminal Justice and University of Texas Medical Branch in 1994, they began a telemedicine program in prisons, in these twenty years they have done 354.000 teleconsults and they have saved around 1.215$ billion until 2010, with teleconsults, EMRs, Teletraining program.



It is a clear example where We can to obtain economical rentability with investments in technology and new process.

Un gran amigo en todas sus ponencias, relativas a la actividad que realizaba al frente del programa de telemedicina del Servicio Extremeño de Salud, decía: "La telemedicina no es un lujo, es una necesidad y yo diría que en ocasiones una obligación.".

El Dr. Claudio Pelaez es un gran defensor del uso de la telemedicina y durante los diez años que ha estado al frente del programa de telemedicina del Servicio Extremeño de Salud, ha sabido desarrollar su uso, en todo lo que ha podido o le han permitido. Este programa de telemedicina comenzó su andadura por el año 2002 con su primer piloto y poco a poco aprendiendo a base de ensayo-error, se fue configurando y desarrollando un protocolo de trabajo que ha generado experiencia y un modelo asistencial en telemedicina contrastado y que es satisfactorio para los usuarios en aproximadamente un 90%, gracias a un fenomenal equipo de profesional que siempre ha colaborado.

Una de las apuestas que, desde la Junta de Extremadura, Servicio Extremeño de Salud y profesionales relacionados con la red de telemedicina, fueron más interesantes, fue llevar la telemedicina a un grupo de población con grandes dificultades de acceso al sistema sanitario que es la población reclusa en prisiones y no derivadas de la distancia física ya que las prisiones de Badajoz y Cáceres se encuentran ambas próximas a los núcleos urbanos más importantes y poblados de la Comunidad Autónoma, sino por las dificultades que genera el transporte de recluso, debido al uso de vehículos especiales, escoltas policiales, medidas de seguridad elevadas, etc..



De igual modo en el año 1994 el Departamento de Justicia Penal del estado de Texas (TDCJ), junto a la Universidad de Texas (UTMB), desarrollaron su programa de telemedicina con unos datos que arrojan unas 354000 teleconsultas realizadas en estos casi veinte años, que han generado unos ahorros estimados de 215 millones de dolares entre 1994-2000 y 1000 millones de dolares entre 2000-2010, estos ahorros no son achacables al 100% al uso de la telemedicina, sino también influyen factores como uso de formularios comunes, Historiales Clínicos Electrónicos (HCEs) y programas educativos entre pacientes y doctores, etc.., pero la teleformación, el uso de HCEs, etc.., también puede ser considerada una parte de la telemedicina y no solamente las teleconsultas clínicas.



Un claro ejemplo que invirtiendo en tecnología y con un buen uso, se pueden obtener datos muy importantes de ahorros económicos, tan a tener en cuenta en situaciones como las actuales. La telemedicina como siempre dice Claudio Pelaez, no es un lujo, es una necesidad y con estos datos de ahorro porque no una obligación.

Our condolences to Galicia

What can I say? It is very sad huge loss of so many lives in a tragedy of such a level. Unfortunately in Galicia is not the first time that we live an unprecedented tragedy, Prestige and the tar, summer fires of 2006 and now this horrible train accident.

I know the strength and tenacity of the Galician people, and I have shared with them a few years of my life. And now that I find many hundreds of miles away and with an event of this caliber I can only offer condolences to all the families of the fatalities, wish the best to all injured survivors and recover as soon as possible and big hug and all my support and encourage my friends and all the people of Galicia, which they again puts the knee on the floor, but I know that they going to be stand up again.

Luto por Galicia

¿Qué puedo decir? Es una pena enorme la perdida de tantas vidas en una tragedia de tal nivel. Por desgracia en Galicia no es la primera vez que vivimos una tragedia sin precedentes, el Prestige y el chapapote, los incendios del verano del 2006 y ahora este horrible accidente ferroviario.

Conozco la fuerza y el tesón del pueblo gallego, ya que he compartido con ellos unos cuantos años de mi vida. Y ahora que me encuentro a muchos cientos de kilómetros de distancia y ante un hecho de este calibre solo me queda dar el pésame a todos los familiares de las victimas mortales, desear lo mejor a todos los supervivientes heridos y que se recuperen lo antes posible y un abrazo muy grande y todo mi apoyo y animo a mis amigos gallegos y a todo el pueblo de Galicia, que otra vez pone la rodilla en el suelo, pero que se con total seguridad que se levantará de nuevo.

The Spanish Government wants to save 558 million euros in three years, implementing the "e-Health"

Among the measures proposed and included in the Reform of the Administration, find 22 on the Ministry of Health, Social Services and Equality, 12 of them relating to the Ministry of Social Services and Equality and 10 relating to the General Secretariat of Health and consumption, the ultimate object of these measures is to avoid duplication and simplify administrative procedures to strengthen cooperation mechanisms of the different public administrations.

From Extremadura must indicate that these three issues "core" and take time to operate and effectively represent a cost savings and an improvement in the quality of health care. But from Blogtelemedicina We miss a boost to telemedicine tool that would increase even more if possible savings and provide healthcare of the highest quality, especially in areas with less population density.

While it is true that the concept of eHealth is very broad and includes concepts such as electronic prescriptions, electronic medical records interoperable electronic health card, not least the use of telemedicine and telecare at all levels, which is a new model of care, which will require changes in the way of actual work,  because it allows a patient who is in a remote area and thanks to the possibilities that today's technology allows, to make a videoconference and a doctor can give a diagnosis. We see telemedicine as something completely normal and usual, like today We are using the telephone or other means of telecommunication.

In Extremadura we performing it since 2003 and as of today if you ask any patient who has been treated by telemedicine on its usefulness, you'll see that immediately responds that it must be something implemented at all sites. It would be interesting to conduct an economic study of the savings during this period and encourage to SES(Servicio Extremeño de Salud) to perform it.
