Telemedicine is health to distance like concept, thanks to technology and communications. It isn´t a promise or any futuristic idea. Today telemedicine is more a reality.
For Cisco, in Costa Rica this trend, that it is a reality in other countries, has begun to win terrain thanks to high bandwidth of Internet and high quality video codecs.
A research done by Cisco with Princeton Survey Research Associates International, in sixteen countries, at 2010 show that, more important worries for telemedicine leadership companies are, allow equitative access to healthcare services, efficiently and with high quality. This study detected like primary problems, low numbers of specialists and its bad distribution, patients bad referred to hospitals and waiting list very long.
Now exist a crisis in healthcare services, not only in Costa Rica also in some countries around the world, some of them with high development like Spain. And telemedicine helps to go out of it. Allow remote healthcare helps to reduce the congestion in medical centers and finally it benefits to patients, says Ariel Szternberg from Cisco.
These teleconsults might benefit to old people and any person that can´t go to medical center by distance, geography, etc. and it needs one specialist that only is in other place. All these things are possible thanks to telemedicine. For us, our next step will the telediagnostic not only with vitals also with electronic health records complete with images, clinic exams, all them digitilized and sent by Internet, says Ariel Szternberg.

Telemedicine Course in Perpetuo Socorro Hospital at Badajoz.
May 15 and 16th, In Perpetuo Socorro Hospital at Badajoz, the Escuela de Ciencias de la Salud in Extremadura, has did a telemedicine course titled Telemedicine Course for Primary healthcare and Speciality healthcare about telemedicine and its use in Extremadura.
SES has bet by telemedicine since 2002, when its telemedicine pilot
program was a success, and they decided to investment in telemedicine for the future. Today in Extremadura there are 47 telemedicine stations, around all its territory in 30 Health Centers, 13 Hospitals, 2 Prisons, 1 Old People Center and 1 Teenage Center. In all this time, ViTelNet and its section in Spain ViTelNet Europe have helped in maintenance this telemedicine network, and broadcast the benefits of a good telemedicine network.
From Blogtelemedicina encourage to the SES will continue on this path, develop a more widespread use and broadcast the benefits that telemedicine is bringing in Extremadura.

Telemedicine in Asturias.
Asturias has presented Thursday, April 19, at the Committee of the Regions of the EU's its experience in telemedicine in the Health Area I of the Principado de Asturias, with head in the hospital Jarrio in the framework of the Health and Telemedicine Network rural (RESATER) and leaves the surrounding Asturian in an advantageous position in Spain in regard to the implementation of new technologies in the healthcare system.
This initiative, developed jointly by the Goberment of Asturias and the Asturian Federation of Councils (FACC) was presented at the Workshop 'e-health: what solutions arise for the future of Europe's rural areas?', With which the Committee of the Regions in Brussels decommissioning projects promoted by the Territorial Cooperation Programme European Space Southwest.
The experience that Goberment of Asturias presented in this seminar focuses on a program for patients in rural areas can be made from their own health centers screening of diabetic retinopathy, a disease is the leading cause of blindness among patients with diabetes among 25 and 65. The project spokesperson Crisanto Alonso, noted that telemedicine enables early diagnosis, lowers health care costs and a savings to the patient, who does not have to travel to hospital when living in scattered areas of rural Asturias.
Before the start of this program, patients in the health area should be periodically these tests had to move to the Regional Hospital of Jarrio, what is now avoided. After application of telemedicine, 85 percent of the 952 patients enrolled in the program in the Northwest could perform the tests required in their respective health centers without having to go to the hospital. The reduction of the revisions is 42.5 percent, a saving which is estimated at around 60,000 euros.
The methodology of the program is that a medical team with medical instruments goes to health centers, where they took fundus photographs of patients who have to do the test.

Orange and Sanofi show the new telemedicine platform diabeTIC.
Orange and Sanofi have showed the telemedicine platform called diabeTIC during The Annual Congress of Sociedad Española de Diabetes, in Vigo (Galicia).
This telemedicine solution allows to send datas, secure storage of patient's clinical information and service two-way communication between patients and healthcare professionals. So, it makes available to chronic diabetes patients, the new technologies, diabeTIC contributes to greater patient involvement in managing their disease, and that from this point the patient will automatically send glucose values , manually send other variables, exchange messages, respond to questionnaires, access to the library and a reminder alert system.
The goal of diabeTIC is to innovate in the model of diabetes management, raise tangible improvements in the quality of life for patients and assist the clinician in relation to the diabetic patient, while it allowing to increase productivity of health care, optimize resources and control costs.

Medical trains.
There are already several examples in the world of health trains whose mission are to bring health and social care to remote and difficult access areas.
One example is Dr. Ramon Carrillo health train under the Ministry of Social Development of Argentina, which is a creation of the Minister Alicia Kirchner and Secretary Sergio Berni. The train assists small towns with 800 people. who have the hospital to 300 kilometers, so we await the arrival of the train, it is composed of 1 tank, 2 freight cars and 6 cars adapted with dental offices, ophthalmology, radiology, gynecology, laboratory fully equipped with 68 professionals on board.
Train health and social development of Argentina
We can imagine that telemedicine possibilities in these situations and how health professionals can take advantage of these technological tools to bring a better health and better quality to those areas.
Another interesting example is developed in Russia telemedicine system "health train" using telemedicine systems to cover with satellite broadband link all along the way.

Seven trends that mark the future of telemedicine, according to the American Telemedicine Association (ATA).
Jonathan Linkous, executive director of the ATA, has shared seven market trends that will shape the telemedicine and telehealth in the near future. From Blogtelemedicina after reading them, We want to bring them to you, What is your opinion about it?, We would like this to become an open forum about where telemedicine and its future.
1- Changes in the way of payments: According Linkous, the payment models have been the biggest problem of telemedicine in North America, but says that the rapid growth of models such as ACO's (Accountable Care Organizations) and Medical Home model, which are some new models of healthcare, are changing the way to pay for telemedicine services. Based on these data, the increase of the population covered by medical insurance allows you to define new models of payment. So as healthcare services are extended to regional or national health organizations.
2 - Telemedicine as a standard of care: Medical Imaging, X-rays and CT scans digital originate since decades. Teleradiology is now so common that provide radiology services 24/7. The technology can be the foundation for that telemedicine will become a standard service model. From Blogtelemedicina could extend this work to the Teledermatology also, today with existing devices could be a reference to a model in providing standard healthcare.
3 - Appearance of remote medical aid companies: The outsourcing of medical image interpretation is currently used by most hospitals in the United States. If a hospital can outsource radiology services, why not outsource dermatology, neurology, or other clinical services? A number of companies can to emerge to provide remote clinical consultations with medical specialists.
4 - The rise of virtual medical centers: In October, "Mercy Hospitals" announced the construction of a new hospital with a cost of 90 million dollars, dedicated to the "virtual healthcare". This center will have clinicians who see patients from peripheral centers in the four states in which this organization operates.
Since Blogtelemedicina We know also of the ambitious project of Cisco to undertake the construction of a hospital designed with all the technological advances in telemedicine and videoconferencing called Palomar West Medical Campus in San Diego California. Then we leave you a sample of this project known as "The Hospital of the future."
5 - mHealth: mobile health (mHealth) is an important aportation to the suite of technologies that are changing the way medical care. However, in general opinion, the mHealth environment is full of promise that yet to be tested and uncertainties about how to pay for these services is very high.
What do you think about it?. Is this a really new era, We left the eHealth and We enter in the mHealth?.
6 - Organization centralized or distributed: The Federal grants have helped establish over 200 telemedicine programs in the United States, and most seem to present a model in which a large central hospital provides services to remotes sites connected to it; but there is an emerging alternative, multiple centers linked together, that providing or receiving services from any of them to any other point in the network; the Arizona Telemedicine Program and the Ontario Telehealth Network are examples of this new approach.
This new approach is also being considered in Extremadura when updating its network of telemedicine, because experience has shown that telemedicine is not only teleconsultations, there is also e-learning, clinical sessions, etc..
7 - Multi-nationalization: Telemedicine can be an important source of international finantial trades. Global investments in high speed networks, and standardization of clinical practice, are providing the necessary basis. Different issues such as payment mechanisms, trade protectionism and cultural prejudices, remain a barrier, however, the potential revenue from such services could have a significant effect on trade balance worldwide.
Maybe it's time for chances in certain organisms, time for Healthcare Services beyond political boundaries, that they can sign agreements or collaboration agreements with private companies, nongovernmental organizations and even countries without the resources necessary for to have trained health professionals in all specialties.
From Blogtelemedicina We are aware that these ideas come from a country where health policies are very different from European ones, but we understand that is a model that works in some countries and if we make a serious analysis of the different models of healthcare We really think that together We can to find the best option, We being aware that the European model is clearly superior, we can reach create a third option which allows to develop both models and bring them to a higher level.
What is your opinion? What can you tell from your experience, if we plantearamos find a new model of care for telemedicine? Do you think that telemedicine has a future or simply see it as something of geeks?. We want to know your opinion and together try to build a new care model, where telemedicine has full presence, since we are fully convinced that it is useful and can provide a much higher level of healthcare if it well used.

The Department of Agriculture provides more than U.S. $ 6 million for telemedicine projects.
The U.S. Department of Agriculture subsidize a total of $ 6 million in different telemedicine projects for rural areas in the United States. These projects will be held in
six states in the Mississippi Delta region.
The increased investment around $ 3 million will go to building an emergency center in Mound Bayou.
Other telemedicine projects to receive funding are:
- Delta Health Alliance for its development project connecting five acute care hospitals with the Intensive Care Center at the University of Mississippi.
- Sl Foundation for her project on tele-health care in the state of Illinois.
- Tombigbee Health Care Authority in the state of Alabama for its telemedicine project.
- Building Healthy Communities in Louisiana for a home telecare project.
- Oschsner Clinic Foundation in Louisiana for its telemedicine program and Telestroke.
- University of Kentucky for its project to link the hospitals of the Kentucky Telehealth Network.

Dennis Ritchie creator of C programming languange have dead last weekend.
The computer world has lost one week to another of its great innovators, the developer of the C programming language and co-creator of the Unix operating system, Dennis Ritchie died last weekend at age 70.
This engineer graduate in physics and mathematics at Harvard University left a broad legacy that is part of modern computing, C programming language, one of the most used and then base for a multitude of languages as Java. Also He was the creator with Ken Thompson's Unix operating system in 1969. In this system portable, multitasking, multiuser have arisen since other variants such as Solaris, Linux or Mac OS X.
Robert Pike, into his Google +: "I´m confident that there are people who appreciate the scope of their contributions and mourned her death properly. The world has lost a mind truly great," he said about co-creator of Unix.
Dennis Ritchie, who in 1983 was awarded the Turing Award (equivalent to the Nobel Prize of Computing), spent most of his career at Bell Laboratories of AT & T (later Lucent and now Alcatel-Lucent).

October 5, 2011. "Find what you love."
From Blogtelemedicina We want to remember one of the pioneers of the computer world, which unfortunately on the last night left us forever, Steve Jobs. For this reason below We leave the speech that Steve Jobs gave the June 12, 2005 at Stanford University who is regarded as an example of speaking, for its style but above all for the emotional burden of their content, full of humility, nothing to do with the pompous presentation that relied on the release of each of their technology products.

"Me siento honrado de estar con vosotros hoy en esta ceremonia de graduación en una de las mejores universidades del mundo. Yo nunca me licencié. La verdad, esto es lo más cerca que he estado de una graduación universitaria.
Hoy deseo contaros tres historias de mi vida. No es gran cosa. Sólo tres historias. La primera trata de conectar puntos. Me retiré del Reed College a los seis meses y seguí yendo de modo intermitente otros 18 meses más antes de abandonar los estudios. ¿Por qué lo dejé? Comenzó antes de que yo naciera. Mi madre biológica era una joven estudiante de universidad, soltera, que decidió darme en adopción. Ella creía firmemente que debía ser adoptado por estudiantes graduados. Por lo tanto, todo estaba arreglado para que apenas naciera fuera adoptado por un abogado y su esposa; salvo que cuando nací decidieron en el último minuto que en realidad deseaban una niña. De ese modo, mis padres, que estaban en lista de espera, recibieron una llamada en medio de la noche preguntándoles: "Tenemos un niño no deseado; ¿lo quieren?". Ellos contestaron: "Por supuesto".
Cuando mi madre biológica se enteró que mi madre nunca se había graduado en la universidad y que mi padre tampoco tenía el graduado escolar se negó a firmar los papeles de adopción definitivos. Sólo cambió de parecer unos meses más tarde cuando mis padres le prometieron que algún día iría. A los 17 años fui a la universidad. Ingenuamente elegí una casi tan cara como Stanford y todos los ahorros de mis padres, de clase obrera, se fueron en la matrícula. Seis meses después yo no había sido capaz de apreciar el valor de su esfuerzo. No tenía idea de lo que quería hacer con mi vida y tampoco sabia si la universidad me ayudaría a deducirlo. Y ahí estaba yo, gastando todo el dinero que mis padres habían ahorrado durante toda su vida. Decidi retirarme y confiar en que todo iba a resultar bien. En ese momento fue aterrador, pero mirando hacia atrás es una de las mejores decisiones que he tomado. Prescindí de las clases obligatorias, que no me interesaban, y comencé a asistir irregularmente a las que sí consideraba interesantes.
No todo fue romántico. No tenía dormitorio, dormía en el suelo de las habitaciones de amigos, llevaba botellas de Coca Cola a los depósitos de 5 centavos para comprar comida y caminaba 11 kilómetros, cruzando la ciudad todos los domingos de noche, para conseguir una buena comida a la semana en el templo Hare Krishna. Me encantaba. La mayoría de cosas con las que tropecé, siguiendo mi curiosidad e intuición, resultaron ser posteriormente inestimables. Por ejemplo, en ese tiempo Reed College ofrecía quizás la mejor instrucción en caligrafía del país. Recordar que moriré pronto constituye la herramienta más importante que he encontrado para tomar las grandes decisiones de mi vida.
